Supported Living

LifeGate’s Supported Living services will provide individuals with a variety of individualized services to support living independently in the community.

Individuals will Learn self-direction and problem-solving related to performing activities of daily living and instrumental activities of daily living required for the participant to live independently; and engage in community based activities of their participant’s choosing.

The Supported Living services enables the participant to live in a home of his or her choice located where he or she wants to live; and Live with other persons of his or her choosing (not including relatives, legal guardians, or legally responsible persons.
