Welcome to Life Gate Healthcare Services, we are a trusted provider of residential services, day programs, and support services for individuals with developmental disabilities.
We work closely with our clients, their families, and other healthcare providers to develop customized care plans, and we strive to promote inclusion and participation in community activities and events.

Yes. Life Gate accepts referrals for individuals in self-directed services as well as traditional services. Choose “Self-Directed Services (SDS)” on the referral form, or call 443-660-7733.
Day Habilitation, Community Development Services, Employment Services, Community Living Group Homes, Supported Living, Personal Support, Respite Care, Nursing Support Services, and Transportation for individuals with developmental disabilities.

No more than three individuals live together, in modern homes or apartments in safe neighborhoods.
Yes. Life Gate can provide respite care 24 hours a day, 7 days a week, subject to the hours approved and documented in the person-centered plan and agreed by the team.
Services are planned with the individual, family, and Coordinator of Community Services through the Person-Centered Plan. Nursing Support Services require preauthorization from the Developmental Disabilities Administration (DDA), documented in the plan.
